South Hill Foot & Ankle Clinic is a friendly, service-oriented, and outcome-based podiatry practice, and we look forward to providing you and your family with effective foot care services.

At our Spokane office, we treat patients with both conservative and surgical methods (often minimally-invasive). The goal is always to use nonsurgical options first – including orthotic and laser therapies. Conservative care is often successful, but you will be pleased to know Dr. Borys Markewych specializes in minimally-invasive surgeries, which are better than traditional procedures because they have fewer complications (swelling, pain, and infection), no internal orthopedic hardware like pins, screws, and plates, and faster recovery times. Even better, our surgical center is located right in our office (no need to go to the hospital).

Before Your First Visit

 

1. Make sure you have your Insurance Card on hand.

Do to the fact that there are many different insurance providers, many of which are constantly changing it is important that you bring your insurance cards with you when you to your first appointment. If your insurance requires a co-pay, payment is due at the time of the visit. We accept cash, checks, visa, and master card.

South Hill Foot & Ankle participates with several insurance providers, and you can find a list of accepted insurance providers on this page.

2. Fill out the New Patient Forms

To make your appointment the best possible experience, our Patient Forms are available online for you to print and fill out in advance of your office visit. Doing so shortens the amount of time you will have to wait before seeing Dr. Markewych.

The Patient Intake form includes personal and family information needed to establish your file, as well as your current podiatric health status. Since many general health conditions can affect your feet and ankles, the medical information you provide for us will allow us to care for you as a “whole person” rather than just a pair of feet. This form asks for a list of your prescription medication with dosages, which may be brought in as a separate list if you prefer.

With the Notice of HIPPA Privacy Practices form, please read and return the last page of this document at the time of your appointment.

  1. Print out the forms.
  2. Fill out the forms.
  3. Bring them with you to your upcoming appointment.

3. Don’t forget your Picture ID 

Remember to bring a form of Picture ID with you to your first appointment. This can be a drivers license, state ID card.

Arrival Instructions

For the benefit of all patients, we ask that you please follow the arrival instructions found below. Doing so helps to keep all appointments on their scheduled time as much as possible. When everyone follows this system, everyone benefits!

  • Please arrive 10 minutes early for your appointment if you have your paperwork filled out.
  • Arrive 20 minutes early if you need to complete the new patient forms in the office.
